Job DescriptionTesfa Social and Development Association/TSDA is a Local Civil Society Organization established in 2000.
It engages on Poverty Alleviation, Health and HIV/AIDS Reduction.
The organization would like to employ qualified and enthusiastic Care and Support Officer.
The post holder will be responsible to facilitate linkage between Community to Health facility and vice versa, Case management for PLHIV in the community, Build capacity of SSWs and CRPs (volunteers), Build capacity of community structures (Community Committee (CCs) & Community Care Coalition(CCCs),Recording and reporting of community services within the program catchment areas.
Major Duties and Responsivities
Facilitate CHCT’s engagement at community level,
Identify cases that are going to be referred to community from health facility together with the health facility provider or case manager collaborate with health facility level case managers in identifying stable and unstable clients
Ensure linkage of clients refer from community to health facility
Provide community-based case management for stable and unstable patients in consultation with the ART centers together with CRPs
Support follow up of clients at community level on appointment spacing
Monitor on Suppression and viral load barrier analysis
provide technical assistance to Case managers, Social service workers and Community resource persons
Provide technical assistance to (CCs) and (CCCs)
Ensure OVCs on comprehensive care provided service based on standard and graduate based on SOP
Use and implement commcare app/mobile app for referral and linkage and case management
Oversee implementation of CHCT in relevant community based activities including stigma reduction, Gender equality, women empowerment that
contribute to improve targeted testing and quality of HIV care and treatment
Job Requirements
Education
Bachelor’s degree in Health Officer, Nursing, Medicine or other health related fields
Experience
A minimum of 4 years of progressive in HIV care treatment services experience in similar settings; at least 3 years experience have to be on PEPFAR/ USAID funded programs for HIV/AIDS, preferably in an NGO setting.
Abilities and Skills
Specialized knowledge: Training in Adherence Counseling, Couple Counseling and Community HIV counseling.
Up to date with latest developments in HIV Differentiated Service Delivery Models.
Experience on previous CHCT and CVC programs highly preferable
Ability to coordinate and support volunteers working in the community
Good interpersonal and communication skill
Skill in MS windows and MS Office packages (Excel, Word, Power point), Web conferencing applications,
Ability to work independently
Language and Responsibilities
Good command of Spoken and written English and Amharic languages.
