Roles and responsibilities of Community Engagement Facilitators

  • Engage community members in planning, implementing and reviewing community related HIV prevention, care and treatment services

  • Work with Health facilities in linking clients to social and economic strengthening services in the community

  • Identify and refer clients to community from health facility together with the health facility provider or case manager. Collaborate with health facility level case managers in identifying stable and unstable clients

  • Ensure linkage of clients refer from community to health facility

  • Provide community based case management for stable and unstable clients in consultation with the ART centers together with Case workers

  • Support follow up of clients at community level on ART appointment spacing and Multi- month ART reception.

  • Participate in community based service delivery model as per the MOH’s guidance on differentiate care model to clients need and preference

  • Facilitate/Provide community based care and support for non-stable patients

  • Assist CWs in screening of TB, STI and Mental health based on the need for such support

  • Provide technical assistance to CWs, Case Managers and ESAs

  • Engage in the CCCs activity and provide technical assistance in their routine tasks

  • Participate in beneficiary identification including assessments of Households for economic vulnerability and food security

  • Receive list
    of families of index cases and apply risk assessment for testing

  • Conduct index case testing at community level both Assisted and unassisted

  • Use and implement CommCare app/mobile app for referral and linkage and case management

  • Work on other job assigned to them by their supervisors and other Association staffs.

    Job Requirements

    Required Qualifications

  • Education: BSC in Clinical Nursing, Public Health or related fields

  • Specialized knowledge: Training in Adherence Counseling, Community HIV counseling and testing. Up to date with latest developments in HIV Differentiated Service Delivery Models.

  • Experience: Minimum of 4 years out of which 2 years experience in providing HIV prevention, care and treatment services preferably at community level.

  • Ability to coordinate and support Case workers working on case management in the community

  • Basic computer skills and data base implementation

  • Afan Oromo language fluency is mandatory

  • Female applicants are highly encouraged
