Job Purpose The Site Care and Treatment Provider (Clinical Mentor) will implement and provide technical expertise in the area of ART for the Department of Defense (DOD) – funded HIV/AIDS prevention, care, and treatment project at the site level. .
Responsibilities:
• Provide the complete range of services to ART (adult and pediatric) clients on a daily basis
• Implement and provide leadership with DFZ staff at facility level in the provision of holistic clinical care of ART clients
• Monitor and manage clients on ART
• Work with ART adherence supporters to ensure clients are adherent and retained on ART
• Work with facility in-charge and other stakeholders to develop QI plans. With support from COH, introduce, sustain and transition customer care approaches with providers using COH'S RECIPE model
• With support from Demand Creation Advisor and COH, use HCD workshops to engage clinicians and beneficiaries in developing feasible plans that address their needs and acknowledge their understanding of challenges
• Provide training and mentorship using Low Dose High Frequency (LDHF) and
ECHO/eLearning platforms to build skills
• Enhance community-facility coordination for effective decentralized services
• Work with MER to observe and document trends related to viral load results for each client and per facility and report cases of
high VL through relevant channels
• Establish a system for daily data review on key indicators which includes reporting anomalies or data spikes to supervisor. Develop responsive action plan where appropriate
• Attend Facility and Provincial led meetings in HIV
• Complete weekly, monthly and quarterly reports as well as participate in DQA activities
• Work closely with and supervise community volunteers
Qualifications:
• Degree/Diploma in Clinical/Medical Sciences Medical Licentiate OR Nurse Practitioner - Diploma/Degree
• 5-6 years' experience in HIV care and treatment
• Membership in Health Professions Council of Zambia or General Nursing Council of Zambia with valid practicing license
• Experience working with the DFZ and/or other military populations/facilities preferred
• Willingness and ability to work long hours outside of 8hrs
• Willingness to work in the field and to support and supervise volunteers outside the facility
• Ability to multi-task efficiently and prioritize activities in support of project goals and deadlines
• Strong interpersonal communication skills
• Ability to work in a team
