Health Strat Project Officer -Longitudinal Care and Linkage Jobs in Kakamega Kenya


Health Strat is a leading Kenyan, public health organization that designs and delivers impactful 360° health system strengthening solutions that are applicable to low and middle-income countries.

Job Purpose:

  • The Project officer-Longitudinal Care and Linkage will provide guidance related to linkages
    through the continuum of testing to enrollment, continuity of care, patient retention
    strategies, and adherence support.

    Primary Responsibilities:

  • Develops and implements a capacity-building and systems-strengthening plan for continuity of care, patient retention, and adherence support, in close collaboration with key partners
  • Institutionalizes continuous quality improvement systems for HTC, continuity of care,
    patient retention, and adherence support
  • Examines patient linkages/referral systems, retention strategies, and adherence support
    mechanisms and leads the development of strategies to address findings
  • Leads the implementation of training and continuous professional development systems
    for HTC counselors, and for health professionals and lay health workers (CHWs, peer
    educators, etc.) involved in continuity of care and adherence support
  • Evaluates and adapts Community Strategy implementation
  • Develops and distributes appropriate job aides and educational material for treatment
    support staff and counselors
  • Represents the organization at stakeholder meetings
  • Supports staff through onsite mentorship and operations discussions
  • Evaluates outcomes of capacity-building activities and shares recommendations
  • Writes longitudinal care activity reports

    Person Specifications:

    Academic
    Qualifications:

  • Bachelor of Community Nursing/Public Health/Sociology or related field
    Master of Counseling and Psychology (added Advantage)

    Professional Qualifications:

  • Registration with the Nursing Council and/or Association of Professional Counselors
    NASCOP Certified HIV Testing and Counseling Professional

    Experience:

  • 4 years’ relevant work experience, 2 years’ experience at supervisory level

    Skills:

  • Interpersonal and communication skills
  • Analytical and report writing skills
  • Computing skills
