An exciting opportunity for a Nurse Practitioner to join the Residential In-Reach Team in a full time or part time fixed term position. We are also interested in hearing from Nurse Practitioner candidates currently working towards endorsement.

About The In-Reach TeamThe Residential In-Reach Team (RIR) is located at the Austin Hospital Campus of Austin Health and is tiered within the Continuing Care Clinical Service Unit. The service is funded via the Department of Health (DoH), Victoria, for the purpose of reviewing people living in residential aged care facilities (RACFs), to provide a timely, person-centred management approach for acute medical conditions.
Service AimsTo support residential aged care facility staff to manage acute care of
unwell residents
To provide an alternative to hospital-based management of acute conditions
To reduce avoidable transfers to the Emergency Department (ED) from residential aged care
Qualifications/Skills/Experience
Registered Nurse (Division 1) with current AHPRA registration
NMBA endorsement as a nurse practitioner (applicants working towards endorsement will be considered)
Post-graduate studies in area of specialty and/or willingness to undertake Masters level study
Extensive and recent clinical experience in Community / Aged care or Emergency Nursing care
Current driver’s license
Advance level of therapeutic management skills
Ability to evaluate practice at an advanced level
Demonstrated excellent collaborative, leadership, teaching, and interpersonal skills.
Demonstrated ability to formulate and implement new models of patient care
Commitment to patient focused care
